 If you provide us with your insurance identification number when you schedule your 1st appointment we will call your insurance prior to you coming to your first visit. This is done so that you are aware of your financial responsibilities. All deductibles, co-pays and/or co-insurances are to be paid at time of service unless other arrangements by the office staff have been made. Worker’s Compensation and Automobile Accident cases are paid at 100% and no payment is due from the patient. Please make sure any Worker’s Comp and Auto Accident claims have been filed with the proper paperwork. Please bring all claim numbers and related papers to your first appointment. Medicare patients have a $124.00 deductible due this year. It is the patient’s responsibility to call on their insurance benefits as they change from year to year. |
